These terms describe the website trial request and optional SMS program offered by EAGLE MARTIAL ARTS LLC (Kaizen MMA) in Mechanicsburg.
Trial Requests
The website lets you request a free one-day trial. Choose a class and preferred date, then provide the adult or parent contact information. A parent or guardian must submit requests for children and teens.
Class choices are preferences. Staff must confirm the class and time; a request does not hold a place. Contact the gym if you need to reschedule or ask about class fit. Membership agreements and training waivers are separate from this website request.
Optional SMS Program
If you check the trial-request text box, you agree to receive nonmarketing texts from EAGLE MARTIAL ARTS LLC (Kaizen MMA) about your trial request, class confirmations, reminders, rescheduling and questions. If you separately check the marketing text box, you agree to receive occasional offers and promotions about martial arts classes and free trials. Each box is optional and unchecked by default. Neither choice is required to request a trial or make a purchase.
You must be 18 or older and may give permission only for a phone number you own and use. A parent or guardian may enroll using their own number for a child or teen trial. Submitting a request without selecting either SMS checkbox does not authorize that category of texts.
Message Frequency and Rates
Message frequency may vary based on your request and follow-up. Message & data rates may apply according to your mobile plan.
Opting Out and Getting Help
Reply STOP to opt out. You may receive a final confirmation of your opt-out. After that, you will no longer receive messages from this program unless you enroll again. Reply HELP for help or contact the gym using the details below.
Message Delivery
Delivery depends on your mobile carrier and network availability. Carriers are not liable for delayed or undelivered messages. Contact the gym directly for time-sensitive questions.
