Information You Provide
When you request a trial, we collect the adult or parent contact name, email address, phone number, program interests and preferred class dates and times. For a child or teen trial, a parent or guardian provides the participant name and age. Please do not submit a child’s personal phone number or email address.
If you choose trial-request texts or marketing texts, we record each separate choice, the disclosure version, the form source and the time your request reaches our server. A trial request does not itself provide SMS consent.
How We Use Information
We use this information to answer your inquiry, review class fit, arrange a trial and manage follow-up. The dates and classes you choose are preferences. Staff must confirm the class; submitting the form does not reserve a place.
Text Messages
Adults and parents or guardians age 18 or older may choose separately whether to receive texts about a trial request and whether to receive occasional offers and promotions from EAGLE MARTIAL ARTS LLC (Kaizen MMA). Submitting a form does not give either permission. Neither choice is a condition of requesting a trial or purchasing a membership.

Service Providers and Sharing
We use Vercel to host the website, Supabase to store trial requests, and GoHighLevel/LeadConnector to manage contact and inquiry information. Market Muscles may receive gym leads for follow-up when connected. Providers may process information to deliver these services, including messages you choose to receive.
We do not sell your personal information. We do not share mobile information with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are not shared with third parties for their own purposes. Providers supporting our SMS service may process this information only to deliver the service.
Security and Retention
We use encrypted connections and restrict access to trial information to staff and providers supporting our services. No method of storage or transmission is completely secure.
We retain information as needed to handle your request, maintain applicable customer records and meet legal obligations. Contact us to request access, correction or deletion. Some records, including opt-out records, may need to be retained to respect your choices or meet legal requirements.
Website Requests and Other Sites
Our hosting provider processes technical information needed to serve the website, such as your IP address and request details. Links to other websites have their own privacy policies. This policy does not replace policies for separate payment, membership or attendance services.
Website Analytics
We use Google Analytics to understand visits, trial form interactions and successful trial requests. Analytics uses cookies and technical information about your browser. We do not send the names, email addresses, phone numbers or child details entered in our form to Google Analytics. Advertising personalization is disabled. You can limit analytics using your browser settings or an analytics blocker.

We may update this policy when our services or practices change. The date above identifies this version.
